05 M7 Clutch issue

i got an 05 M7. when riding this weekend, seemed to have blown a belt. when i went to go replace it in the field and started to compress the secondary, the secondary just fell off in my hand. the bolt holding it on just vibrated loose. has anyone ever had this problem before?

I had this problem but only with the bdx deflection adjuster and that was because it didnt work the way they said it did or I still do not know how to put it in so I put it in the way I thought it should go in and it worked does not come loose
