Voltage Regulator Failure Causes

I'm trying to understand why so many VR's go down on the Polaris IQ's? I'm aware that some VR's were recalled/warrantied for some reason and others go out because of wire chaffage. If all wires are routed properly and you have a VR that is not part of a bad batch, what else causes them to blow and possibly take out an ecu? Is a spare VR a must with these sleds?
