Arms went past the end of the Ramps?

Helping a buddy with his 04 rev and his clutch locked up! He had a Arm and roller that went past the end of his 415 ramp?? Took it off and apart and the ramps and rollers are fine, busings are good, would a worn/weak spring cause this?? Help Please!

The clutch bolt came loose and backed off.
